Creating a New Registers and Forms Template
- Select on the Admin Gearwheel at the top right of your Assurance Screen and select Registers & Forms Templates.
- Select on +Create Registers and Forms Template button.
- Enter the Name of your Register or Form and select the Create button.
- Select the Activate button to ensure that the new template is accessible. You may Deactivate the template to toggle accessibility, or you can Delete the template if no forms have been submitted.
Note: Deleting the template will permanently delete it from the system.
Setting Up a Registers and Forms Template
Further set up of the template is done on several tabs. Below is some key information that is needed.
- Details - basic template information
- Workflow - management of the stages and approvals
- Fields - management of the fields
- Notifications - management of the notifications
- MergeDocs
- Reference Documents
- Recommended Tasks
- Recommended Checklists
- Public Link - management of the template availability to external users, so they can submit data into the system from a public link, which can be activated here - this is what makes a register a form
- History - provides information around template changes
Please Note: You will need to ensure that whoever is managing the register going forward, has access rights to the register.
Setting Up the Details of a Registers and Forms Template
Registers and Forms Details | Description |
---|---|
Name |
The name of the template. |
Prefix |
The prefix forms the first part of the key for Form created from this template. For example, if the prefix is CON the first Form created from this template will have the key CON-1, then the second will have CON-2 etc. |
Category |
The category of the template. Templates in the same category will appear in the same bucket when selecting on Registers and Forms at the top of the screen. |
Status |
Template status. This can be changed by selecting Deactivate above.

Super Sensitive |
Mark the template as super sensitive. For more information on this feature, click here. |
Ability to make Registers & Forms Confidential |
Allows users to mark Forms made from this template as confidential. For more information on this feature, click here. |
Description |
The description of this template. This information will appear on the Registers and Forms Templates list screen. |
Instructions |
Instructions entered here will appear at the top of the page when a user is creating a new Form from this template. |
Notifications Before End Date |
If the template has the built in End Date field added, this notification will trigger. The Notification before the End Date will send the Person Responsible Notifications before the Date specified in the End Date field based on what has been set up for this notification. To change the settings, select on the blue box and you can change the settings for when the notification should be sent. |
Notifications After End Date |
If the template has the built in End Date field added, this notification will trigger. The Notification After End Date will send the Person Responsible Notifications after the Date specified in the End Date field based on what has been set up for this notification. To change the settings, select on the blue box and you can change the settings for when the notification should be sent. |
Ability to create Register and Forms by Import |
Allows Full Users without Admin Rights to create Forms on this template by import. Note: Administrators will always have the ability to import Forms via the Administration settings. |
Credentials and Eligibility Criteria |
Set up credentials and eligibility criteria for entities on this template. For more information on this feature, please refer to our Support Hub article on How to Set Up Credentials and Eligibility Criteria in Assurance. |
Enable Private Link |
Activates the Private Link for this template. For more information on this feature, please refer to our Support Hub article on What is a Private Link Form in Assurance. |
Top Bar under the Details Tab of a Registers and Forms Template
Along the Top Bar under the Details tab, you will find the following buttons and options.
Top Bar Buttons | Description |
---|---|
Edit | Change the details for this register template in Edit Mode. |
Copy | Make a copy of this register template. |
Export Template | Export out a JSON file of this template, which you can use to import the template into another Assurance site. For more information on this feature, please refer to our Support Hub article on How to Export a Register Template from Sandbox and Import into Live Site in Assurance. |
Activate/ Deactivate | Deactivating means that you will no longer be able to create Forms using this register template, and any Forms that have already been made will become inactive as well. |
Delete Permanently |
Delete the register template permanently. This can only be done if no Forms have been created using the register template. The option to delete a register permanently will not be visible until you have deactivated the register. Therefore, you must click the Deactivate button before you are able to click the Delete Permanently button. |
